How to Request a Home Visit

Home visits are only available for patients who are housebound because of illness or disability or are currently too unwell to attend the surgery.

To request a home visit please complete an Accurx on behalf of the patient. You can do this as a guest, or you can create an account and add someone else. If you complete an Accurx by 10am we will contact you before 1pm and arrange a visit if medically necessary.

If the patient is very unwell and requires attention within the hour, please call reception or 111/999.

We keep a register of patients who are chronically housebound and aim to visit them at least twice a year.**

Our health care assistant Brian and our nurse Monica also do home visits when appropriate.

 

Coronavirus

Due to coronavirus we are not routinely visiting patients. Instead we will proactively telephone patients.
